SAFETY & ETHICS CHECKLIST:

Before the fair begins, our safety officers will be visiting each project and will use this checklist to determine if students will be allowed to compete to move on to the regional level.  If students are missing any of the following forms mentioned, or are breaking any of the safety rules s/he will be able to compete in the fair, but will not be allowed to move on in the competitions.  BE PREPARED!

Ethics & Safety Checklist for the SD23 Science Fair (February 12th 2013)

Please Note: The SD23 District Science Fair follows the policies regarding ethics and safety for the Canada Wide Science Fair. To view the CWSF policies please refer to the 2013 CWSF: Guide at http://cwsf.youthscience.ca/fairs/current

Animal subjects:  If YES, form completed and at station (Form 4.1C)

Human participants: If YES, form(s) completed and at station (Form 4.1B - for SIGNIFICANT RISK or Form 4.1A - for LOW RISK)

If a survey was done, students must have:
1. Copy of the Survey
2. Copy of Letter of Information
3. Sample Informed Consent form (signed forms stored in a private secure place)

Display area DOES NOT include any of the following:

•     More than 1 litre of total quantity of acceptable liquids (refer to the CWSF regulations for acceptable liquids)

•     Flammable, toxic or dangerous chemicals

•     Prescription drugs and over-the-counter medications

•     Biological toxins

•     Cell or tissue samples including blood and blood products, except on sealed microscope slides

•     Plants or plant tissue

•     Soil containing organic material

•     Cultures

•     Live animals (whether microorganisms, non-vertebrate and vertebrate)

•     Radioisotopes or compounds containing radioisotopes at activities above normal background

•     Firearms, ammunition, dangerous goods or explosives

•     Images of humans or animals that have been injured by the use of firearms or explosives

•     Compressed gas cylinders

•     Wet cell batteries

All electrical equipment used in the display is CSA approved.

All participants in images displayed have given appropriate prior written permission.

Exhibit is sturdy, self-supporting and sufficiently stable to prevent accidental tipping.

Sharp edges or corners of prisms, mirrors, enclosures and glass or metal plates that may be contacted by the public are removed or protected to prevent injury.

Dangerous moving parts such as belts, gears, pulleys and blades are provided with a guard to prevent access to the moving parts.

An in-running nip hazard of any part of a motor, device or thing that may be a danger is guarded to prevent contact with the pinch point.

Moving exhibits (e.g., radio-controlled vehicles, robots) shall be restricted to the regulation display space.

X-ray or hazardous radiation-producing equipment is not being operated.
